ALCOHOL & DRUG EVALUATIONS

    

If you are arrested for DUI, IL requires that you complete an evaluation before you are sentenced for the DUI offense or before the Secretary of State can grant you driving privileges.  

PURPOSE OF EVALUATION

The purpose of the evaluation is to determine the extent of the defendant’s alcohol and/or drug use and its associated risk to current or future public safety.   

HOW EVALUATION IS PERFORMED

The State of Illinois uses a DUI Alcohol and Drug Evaluation Uniform Report to determine the extent of alcohol/drug use and it's associated risk.  The report consists of many factors:  

  • Objective test score
  • Driving history and current offense
  • Chemical test results ( blood alcohol concentration – BAC)  from past and current arrests
  • Initial screening interview with an evaluator who is a certified alcohol and drug counselor


The screening interview will obtain facts including:

  • Past and current alcohol and drug use
  • Past and current arrests involving alcohol and drugs
  • Determination of the extent to which the substance use has caused marital, family, legal, social, emotional, vocational, physical and/or economic impairment


These factors listed above are used to determine a recommended risk level of one of the following:  

  • Minimal
  • Moderate
  • Significant 
  • High

RISK LEVEL CLASSIFICATION

Risk Levels determine the extent of the defendant’s alcohol and/or drug use and potential risk to current or future public safety.   

Risk Level Recommendations are based on what is clinically appropriate to address concerns related to alcohol and drug use.
These recommendations could include:

  • Alcohol and drug education classes
  • Intervention treatment
  • Substance abuse treatment
  • Ongoing activities outlined in continued care plan

WHAT TO EXPECT

Throughout the entire process, a caring and understanding counselor will treat you with respect & dignity, provide a safe and non-judgmental atmosphere and protect your confidentiality.   


When you arrive for your appointment, you will be given a pamphlet explaining the evaluation process and expectations.  The evaluation process:  

  • Fill out paperwork
  • Take ASUDS questionnaire 
  • Evaluation screening interview with the evaluator


After the evaluation has concluded, a classification and a recommendation will be determined by the evaluator and recorded on the Alcohol and Drug Uniform Report Form.  This form will be given to you and sent to the circuit court of venue or the Office of the Secretary of State.

WHAT TO BRING TO YOUR EVALUATION

To complete the evaluation, you need to bring to your appointment:

  • Copy of the ticket(s) and any related paperwork from your DUI arrest(s).
  • Current Illinois Driving Abstract.  The Driver’s Abstract can be picked up at any driver’s license facility or at the Secretary of State location at 17 North State Street, 10th Floor in Chicago. You can also use the following link to get a copy by mail. 
  • Payment for the evaluation.  We accept cash, check, credit card or debit card.  Unfortunately, this service is not covered by insurance.




 If your arrest took place in Cook, Lake or DuPage County - you must attain your evaluation from that court's designated provider unless special circumstances are granted by the court.  We are not a designated provider for these counties.
